Document and Information Checklist
This checklist is a comprehensive list of documents and information that might be relevant to your situation. Don’t feel overwhelmed by the number of items listed! A good first step is to cross off anything that isn’t relevant to you. For example, if you’ve never served in the U.S. military, you can cross off any items related to military service (e.g., DD214). This document will help you identify what you may need to organize for your Power of Attorney (POA) or personal agent..
Documents and Information Inventory
This inventory document looks like the checklist, but its purpose is slightly different. It helps you track where your documents and important information are stored. For instance, if you keep your documents in a safe deposit box, you can use this inventory to let your POA or personal agent know where the box is located and how to access it (e.g., key or code). This ensures they can easily retrieve your documents when necessary.
Health Information Worksheet
This worksheet is designed for your healthcare POA to ensure they have all the relevant medical information about you. It’s important to share this with your healthcare POA so they can make informed decisions on your behalf, especially if you're unable to communicate. For example, medical providers will need to know if you have implants like a pacemaker before performing certain procedures or diagnostic tests.
