The Alzheimer's Association South Dakota Chapter provides a variety of resources and support options to individuals and families affected by Alzheimer's disease or other types of dementia.
Dakota at Home
Dakota at Home is a free information-and-referral service run by the Division of Long Term Services and Supports. They provide objective information and options planning to help individuals identify and access public and private services and supports in their local communities, regardless of age, disability, or income.
Driver Evaluation Request
This South Dakota Driver Evaluation Form can be used if you have concerns about an adult's ability to operate a motor vehicle in the state of South Dakota. Age is not an acceptable reason to request a driver evaluation.
Guardianship and Conservatorship
Guardianship and conservatorship become necessary when an individual is incapacitated and does not have the appropriate paperwork in place (for example, Power of Attorney). Family caregivers can find answers to some of the most frequently asked questions about guardianships and conservatorships in the state of South Dakota in this resource developed by South Dakota Legal Self-Help. Helpline Center’s 211 program offers free resource navigation services for South Dakota residents. You can contact a trained specialist by phone, text, or email to learn more about non-profit and government programs that available for caregivers. Medical Order for Scope of Treatment
Medical Order for Scope of Treatment (abbreviated as MOST) refers to medical directives for patients who have been diagnosed with a terminal condition. The MOST is valid across all medical settings, including medical settings in the home. MOST must be completed by a medical provider.
